Concrete Patio Table Advice

I'm planning to build a concrete table. 4x4s for the legs notched out with 2x4s for support across the length and width and finally 2x4s spanning the width of the table every 6 inches or so. I'm planning to make the concrete table 80 inches long and about 40 inches wide and 1 inch thick. I'm going to use Rapid Set high strength mortar mix, some remesh, and i also have fiberglass I'm planning to throw in the mix. Finally some superplasticizer to make the mix more pourable. Couple questions: Do you think pour in place is the only/best option? What about using a form with melamine and transferring the concrete then to the base? My worry with the form is that this is going to be heavy and I worry it'll crack and/or it'll take a lot of people to lift. My worry with pour in place is I'd like to hide the cement board that is the base and I'm not sure how to do that without plastic forms. I've been trying to print out the forms, but it'll take a bit, and I'm not sure PLA will hold up to the stresses and heat of the curing process. Is there anything glaring that I'm missing? Any words of wisdom or advice to make this easier? TLDR; I want to make a concrete patio table. Any advice to ensure I don't mess it up badly.
